Blank scholarship applications are available as PDFs on-line here. They are also available on request by calling 309-827-5534 (and an application will be mailed to you).

We are unable to accept or process applications on-line. The application indicates the address to which it should be mailed.

For the coming academic year, applications must be submitted by June 1. We consider all applications and every application receives a response letter.
